Legacy Introduction Form

To be used by individual Pi Beta Phi alumnae.

Pi Beta Phi mothers, sisters and grandmothers use the Legacy Introduction Form to inform a Pi Beta Phi chapter that their legacy (daughter, sister or granddaughter) is a student on campus and may be interested in or is planning on going through recruitment.

A Legacy Introduction Form is strictly an informational, optional introduction and does not take the place of a Recruitment Information Form (RIF) required to sponsor an individual for membership in Pi Beta Phi. If the mother, sister, or grandmother has since passed away, a family friend can also complete this form on behalf of those deceased. A collegiate member, who is not eligible to write a RIF, may complete the form to notify another chapter that her legacy sister is interested or is planning on going through recruitment on their campus.

Regardless of whether a Legacy Introduction Form is used, a RIF for that legacy must be mailed to the chapter by an alumna who wishes to sponsor her for membership in Pi Beta Phi.

RIFs and information about the recruitment process can be obtained here or directly from Headquarters at recruitment@pibetaphi.org.

The information provided in this form will be forwarded to the Vice President of Membership for the chapter selected below.

You will receive a confirmation email a week or so after the form is submitted.
