FDS500 Nomination Form 

Book donations are an important component of any Fraternity Day of Service event. Access to books is a critical step in learning to read for all children, especially those who don’t have their own books at home. 

Your chapter or alumnae club has the opportunity to nominate a qualified First Book® recipient group of your choice to receive new books for children in need through a Pi Beta Phi FDS500 grant, made possible by gifts to The Literacy Fund at Pi Beta Phi Foundation. If your nominated group is selected, they will use the grant to purchase 500 new books from First Book Marketplace. Your chapter or club will assist with the book distribution, which can be incorporated into your Fraternity Day of Service celebration in March. 

Please complete the form below for the opportunity to be one of the 100 chapters or alumnae organizations selected to participate. The nomination period for FDS500 grants opens August 30, 2019 at 12:00 a.p. CDT and closes September 13, 2019 at 11:59 p.m. CDT. After the nomination period closes, 100 recipient groups will be selected at random. You will be notified by September 30, 2019 if your group has been selected.
